Your consent will expire upon your termination of your CMT EMR account. You may revoke your consent at any time by notifying CMT. However, any previously disclosed information would not be subject to your revocation request because once the health information is released by CMT it may be re-released by the recipient of the information and no longer protected by federal law.

In order to participate in the EMR, you must agree to the terms and conditions contained in this Privacy Policy, which is a legal agreement to which you are contractually bound.
